Orders are accepted by approved Visa, MasterCard, or American Express credit card (in U.S. funds) with or without additional gift certificate(s) or discount(s) issued by BD.

 

Offline Credit Card Payments: If you prefer not to process your payment online, you must still complete the online ordering procedure. During CHECK OUT, select the "Pay by Check" payment method, and input BY PHONE or BY FAX or BY MAIL in the Check Number field. Then print out and complete the
E-Learning Registration Offline Payment Form and PHONE, FAX, or MAIL the Offline Form to us as instructed.

Registration will not be considered complete, and the E-Learning Access Instructions will not be e-mailed, until your payment is processed and approved.

If you prefer to "Pay by Check," you must still complete the online ordering procedure. During CHECK OUT, select the "Pay by Check" payment option. The dropdown menu will prompt you to input your check number. Then print out and complete the E-Learning Registration Offline Payment Form and MAIL your payment along with your completed Offline Payment Form to the address indicated.

Registration will not be considered complete, and the E-Learning Access Instructions will not be e-mailed, until your payment is processed and approved.

Registrants using a facility Purchase Order (customers within the United States only), must still complete the online ordering procedure. During CHECK OUT, select the "Pay by Check" payment option. The dropdown menu will prompt you to input a Purchase Order number. Then print out and complete the E-Learning Registration Offline Payment Form and FAX or MAIL a copy of your Purchase Order along with your completed Offline Payment Form as indicated.

Registration will not be considered complete, and the E-Learning Access Instructions will not be e-mailed, until your facility PO is processed and approved.
